Step 2: Wait for a Call from Our Team
Once we receive your form, one of our nurses will contact you to schedule your first appointment and guide you through next steps.
Before Your Appointment
After your visit is scheduled, please complete these additional forms. If you would like to fill out the paperwork in person, someone at the reception desk will be happy to assist you. Each of these forms must be completed for your appointment.
What to Expect at Your First Visit
We want your first visit to feel smooth, welcoming, and stress-free. When you arrive at SAMA Healthcare, our front desk team will help you get checked in and make sure your paperwork is in order. Then, one of our providers will meet with you to review your medical history, listen to your concerns, and begin building a personalized care plan.
What to Bring:

- Your insurance card
- A list of current medications
- Any relevant medical records or prior test results
During Your Appointment:
- We’ll review your medical background
- Perform any necessary exams or screenings
- Discuss your current health goals or concerns
- Answer your questions and outline next steps
